1) I say nay.  Just weird them up a bit like you did converting the +1 plate to power armor.
2) Perhaps the keep borders the mutant-filled radioactive desert in the north?  Really any geographical feature that you designate as being more monster-filled will do.
3) All the humanoids become men of various colors, the dregs of their society, united by the sorcerer who runs the Shrine of Evil Chaos (which I would rename if I was you), the Ogre, Minotaur and Meduse become hideous mutants.  I'd probably make the medusa into a woman whose entire head is just a mess of snakes, no human face at all.  Maybe the snakes spit petrification venom.  The skeletons become Terminator robots.
4) Keep the weird hermit guy as is.  Maybe make his pet something different.
5) Sure, sounds perfect to me.
6) Fighters don't need any particular mechanical help, in my opinion.
7) If I recall correctly The Residents' album Hell! had some pretty creepy 'anti-music' tracks that might work.

Ed's idea is basically what I do.  My Wessex campaign is slightly reskinned southwest England, circa 1140AD.  I've got a ton of stuff I can swipe from actual history, but I can also pick and choose because the setting isn't really Historical England, it's Mythic Wessex.

I have a section of my dungeon where one can simply wander into (my version of) Carcosa by accident.  A few of my players have figured out where they end up, but some just know it as "that weird place with the giant ants and cancer wizard".

I'm not sure if the snake men would value a half and half human or not.  After all, they bred the distinct color lines for a reason.  Human sorcerers would probably be extremely curious about such a character, since they lack the theoretical knowledge of the snakies to know what would happen if a half-color person were used in a ritual.

I don't have the new book yet, but IIRC the original volume makes it clear that Geoffrey intended for the colors of men to be non-interfertile.

These freaks have been generated for use with the Summon spell from Lamentations of the Flame Princess Grindhouse Edition, using the procedure on pages 143-148 of the Rules & Magic booklet.  The demons below should hopefully be enough to cover the ref's ass for a session or two should a PC start with Summon in their spellbook.
